Terms, Conditions, and Program Policies
Welcome to Elevate Medical Aesthetics. By enrolling in any of our auto-debit membership programs (including the Elite Beauty Bank, Royal Beauty Bank, The Glow Club, or The Luxe Club), you explicitly agree to be bound legally by the operational policies, commitment lengths, and financial frameworks outlined below.
1. General Structural Rules & Billing Framework
All automated clinical programs operating at Elevate Medical Aesthetics PLLC are strictly bound by the following institutional rules:
- Initial 3-Month Minimum Commitment: To prevent program structural abuse, all membership options carry a mandatory, contractually binding minimum term of three (3) consecutive monthly billing cycles. Memberships cannot be frozen, paused, or canceled prior to the successful clearance of the third monthly auto-draft.
- Automated 30-Day Billing Recurrence: Members authorize Elevate Medical Aesthetics PLLC to securely store an encrypted credit card token on their personal Boulevard profile. This card will automatically be drafted every thirty (30) days for the specified membership tier price.
- Payment Failure & Immediate Suspension: In the event that a recurring auto-draft is declined, all structural member perks—including locked-in VIP toxin pricing, skincare retail discounts, active device a la carte savings, and unredeemed Service Club treatment vouchers—will be instantly suspended. Accrued benefits will remain locked until the account billing profile is successfully updated and past-due invoices are completely cleared.
- Non-Combining Discount Safeguard: Membership discounts represent our premier baseline clinic savings tier. As such, member discounts are strictly non-combinable and cannot be stacked with package specials, limited-time flash sales, event promotions, or external third-party medical reward programs.
2. Automated Click-to-Cancel & 30-Day Notice Policy
Elevate utilizes an entirely automated, user-initiated cancellation system managed directly through your secure Boulevard Client Portal account or mobile app interface.
- The 30-Day Cancellation Window: Cancellations must be initiated through your digital portal profile. Because all plans enforce a mandatory 30-day structural notification period, clicking “Cancel” will instantly drop your account into a “Scheduled Cancellation” state. One final monthly auto-draft will process normally on your next scheduled billing date before the system officially updates and closes your plan.
- Past-Due Cancellation Blocks: If your billing profile has a status of “Past Due” or features an un-cleared failed invoice, the automated system will prevent online cancellations. Accounts must be made completely current before a scheduled cancellation can be successfully processed by the software overlay.
3. Credit-Based Beauty Banks (Elite & Royal)
Our Beauty Bank models act as credit accumulation frameworks to assist clients in budgeting for yearly advanced cosmetic maintenance.
- 100% Cash Value Accrual: Every dollar drafted from your billing method stays completely yours, accruing safely as a spendable account balance to apply toward any individual clinical service, package, or professional retail product.
- Strict Non-Refundable Nature: Banked dollar balances represent pre-purchased internal clinical credits. Under no circumstances are accrued Beauty Bank funds refundable, withdrawable as cash, or cash-equivalent values back to a credit card or banking method.
- Post-Cancellation Forfeiture of Benefits: If you choose to terminate your Beauty Bank program, any accrued cash credits remaining on your clinical profile will remain safely yours to spend. However, upon the moment of membership cancellation, all preferred member toxin rates and retail tier discounts are immediately and permanently forfeited. Any remaining funds will be spent strictly at full, standard a la carte retail pricing.
- Gifting & Guest Boundaries: You may utilize your personal accrued Beauty Bank cash balances to check out or purchase treatments for family or friends. However, any guest treatment checkouts are handled strictly at full standard clinical retail rates; member-exclusive pricing and tier percentage discounts are completely non-transferable.
⚠️ FILLER & DEVICE RESTRICTIONS: Royal Bank dermal filler savings apply strictly to a single individual syringe of Radiesse® (+) or the Teoxane RHA® Collection per visit (Belotero Balance® is exempt). A la carte device and microneedling savings apply strictly to single facial or body skin sessions; treatments for scalp hair loss, alopecia restoration, packages, or PRP/Exosome upgrades are completely excluded.
4. Value-Based Service Clubs (Glow & Luxe)
Our Service Clubs act as highly incentivized skin and grooming conditioning passes engineered around advanced device workflows.
- The “Use-It-or-Lose-It” Policy: Monthly treatment choices and service vouchers granted within The Glow Club ($199/mo) and The Luxe Club ($349/mo) exist on a strict, automated 30-day validation cycle. Unused monthly treatment choices do not stack, do not roll over to subsequent calendar months, and cannot be salvaged or stored.
- Immediate Post-Cancellation Voucher Eradication: Once a 30-day cancellation sequence finishes processing and the account officially closes, any unredeemed treatment vouchers or pending monthly choices left sitting on the user’s profile are instantly wiped by the system. Cancelled vouchers are completely void, cannot be reclaimed, and hold zero cash-redeemable value.
- Strict Non-Transferable Rule: Monthly club service allocations can only be rendered to the primary account holder. Club members cannot transfer, gift, or assign their monthly treatment selections or baseline retail perks to any other client profile.
- Skincare Line Specificity: The unlocked product percentage discounts apply exclusively to in-office, physical retail inventory from our authorized clinical formulations: Skinbetter Science®, SkinMedica®, and DP Dermaceuticals. Online corporate portals or unlisted cosmetic brands are fully excluded.
